Returning students:
Parts 2 & 3 have retained past medical information. Review, update if necessary, and sign off.
6th graders must submit updated immunization records.
New to Discovery students are required to have a physical on file: Physical Form and submit immunization records.

The Free and Reduced Lunch Program
Discovery Charter School will continue to operate the free and reduced lunch program. The program is more than just lunches. This program also establishes eligibility for such benefits as the reduced/waived fees for the school year.
Meal charge policy and procedure
Students may automatically qualify for Free or Reduced Lunch or may apply for the program. Note: Students who automatically qualify will be contacted via email or letter in August. To avoid disruption of benefits, please fill out an application. Families who previously filled out an application and were approved for the 25-26 school year should fill out a 26-27 application as well to avoid a disruption in benefits.

Fees (transportation and consumable) per student.
Discovery still maintains collection of fees for transportation of Learning Experiences ($150/student) and consumable materials for project-based learning and classroom use ($50/student). These fees will be posted in Power School on September 1st. Please look for further communication once the school year begins.
Additional Information
Sports Physical: If your child plans on participating in an after-school sport, your child will need to have a completed Sports Physical. This form should be given to the coach prior to the start of the sports season. The Sports Physical should not be turned in at Registration and should not be turned into the school nurse. Please be sure to make multiple copies in the event your child would like to participate in more than one sport. The main office and coaches will not be responsible for transferring the sports physical to other coaches.

The Uniform Store is open for ordering here: Uniforms. You can select home delivery, free instore pickup, or free delivery to school. PAC will be selling left-over stock on hand and gently used uniforms at Meet & Greet in August.
